By making this purchase and executing this Agreement or SOF, you are acting on behalf of your organization and it is understood that:

The CPT^® codes are owned by the AMA and Customer is entering into a subscription through Company to use such code sets;
Customer has reviewed the AMA definition of "user" and hereby confirms that this Order accurately reflects Customer's current number of AMA "users";
Customer's use of the code sets authorizes the AMA to audit Customer to confirm the accuracy of Customer's "user" count;

Each subscription commences upon the execution of this order form by Customer and continues until December 31 of the same year (or December 31 of the next year if purchased in December of the previous year). Each subscription automatically renews each January 1st for successive one year terms at the then current rates unless a party provides written notice of its intent not to renew at least 60 days before the end of the then current service term; and,
